This Privacy Policy explains how NOURMDX LLC, doing business as Wellness First Urgent Care (“we”, “us”, “our”), collects, uses and protects information through our website, our patient communications and our appointment text message program.
Your health information
Health information we create or receive while caring for you is protected under the Health Insurance Portability and Accountability Act (HIPAA). How we use and disclose it is described in our Notice of Privacy Practices, available at our front desk and on request. Where this policy and our Notice of Privacy Practices differ about health information, the Notice of Privacy Practices applies.
Information we collect
- Contact details you give us, such as your name, date of birth, mobile phone number, email and address.
- Appointment details, such as the date, time, location and provider of your visits.
- Text message consent records, including when and how you agreed to receive texts, and any opt-out requests.
- Website information, such as pages visited and basic device and browser information.
How we use your information
- To schedule, confirm and remind you about appointments, and to tell you about changes to them.
- To send secure links that let you view or manage your appointment, and one-time verification codes to protect them.
- To answer your questions and provide patient support.
- To operate, secure and improve our services, and to meet our legal obligations.
Appointment text messages (SMS)
If you agree to receive text messages, we use your mobile number only to send the appointment messages described in our SMS Terms. Message frequency varies, up to 6 messages per appointment. Msg & data rates may apply. Reply HELP for help, or STOP to opt out at any time.
We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes.
Mobile opt-in data and consent will not be shared with any third parties, except the service providers who help us deliver our messages, as described below.
How we share information
We do not sell personal information. We share it only:
- With service providers who help us operate, such as our scheduling, patient-communication and messaging providers. They may use it only to provide services to us, under confidentiality obligations and, where health information is involved, a business associate agreement.
- When the law requires it, or to protect the rights, safety and security of our patients, staff and practice.
How we protect information
We use administrative, technical and physical safeguards to protect your information. Appointment links are unique to you and require a verification code before an appointment can be changed. Standard text messages are not encrypted, so we keep them brief and never include test results or diagnoses.
Your choices
- Reply STOP to any text to stop receiving messages. Reply START to resubscribe.
- Contact us to update your contact details or to ask about the information we hold.
Children
Our website is not directed to children under 13. A parent or legal guardian manages communications for a minor patient.
Changes to this policy
We may update this policy. The effective date above shows when it last changed.
